Assistance to strengthen a participant's ability to understand and implement their plan, connect with providers, coordinate informal, mainstream and funded supports, and build capacity over time.

Specialist Support Coordination

Higher intensity coordination for participants facing complex barriers, crisis, significant risks, service breakdown, housing instability, hospital or justice interfaces, or other complex support environments.

Psychosocial Recovery Coaching

Recovery-oriented coaching for participants with psychosocial disability, focused on building capacity, strengthening recovery goals, increasing connection, and coordinating supports around mental health and daily life.
